Forms due to play fall sports

All student-athletes must submit completed blue cards and proof of a current physical to the school nurse to play interscholastic sports this Fall. Please contact your school if you need either form.  Varsity and JV forms are due Aug. 16. Modified forms are due Aug. 22.
